Any idea how it is calculated? I don't think it is total medals or total stars, so what else could it be?

It's medals.

The top alliance has 12k points. 50 members with 500 medals each would have a total of 25k points. It isn't just medals.

It's not total medals combined. It's a discounted rate of total medals.
